    Living with a chronic condition such as diabetes, heart disease, asthma or arthritis can feel overwhelming.     What Is a Chronic Disease Management Plan?

    A GP Management Plan (GPMP) is a structured plan developed by your doctor to help you manage your chronic condition. It outlines your health goals, the treatments and therapies you need, and a schedule for regular reviews. If you have a chronic condition that has lasted, or is expected to last, six months or longer, you are likely eligible for a management plan.

    One of the greatest benefits of a GPMP is that it can give you access to Medicare-funded allied health services. Under a Team Care Arrangement (TCA), you may be eligible for up to five subsidised visits per calendar year to allied health professionals such as physiotherapists, dietitians, psychologists and podiatrists.

    We also focus on preventive care, helping you make lifestyle changes that can slow the progression of your condition and reduce the risk of complications. This might include guidance on nutrition, exercise, stress management and sleep.
